A cosmetic item refers to items, usually associated with armor, in which can either be worn in the appropriate equipment slot or used to transmogrify other items of the same type. Cosmetic items ignore armor types such as cloth, leather, mail and plate.

[Griftah's Authentic Troll Shoes], no longer available to players.

Siege of Orgrimmar

Shaman-only personal looted from [Kor'kron Shaman's Treasure] which drops off of the Kor'kron Dark Shaman (flex, normal and heroic modes). It cannot be bonus rolled.

As of Warlords of Draenor, it can occasionally be purchased from the BMAH with a starting bid of 15,000.

These items require buildings in the garrison.

Battle.net Store

Three helms are available in the Battle.net Store for $15 US or €12, each with a unique look.

Notes

Creating this new type of armor was logically the next step, rather than creating four items to serve the same purpose like what occurred during much of Mists of Pandaria with items such as: [Straw Hat], [Straw Hat], [Straw Hat], [Straw Hat].
