The Cracked Titan Gem drops from Scalecommander Sarkareth in Aberrus, the Shadowed Crucible. It starts the questline to obtain the evoker legendary weapon, 
[Nasz'uro, the Unbound Legacy].
After the first region-wide mythic Sarkareth kill, the gem drop is unlocked region-wide with the next weekly reset, and drops on all difficulties. The drop rate increases the higher the difficulty.[1]
