Cynthia Wallerby was a mage who, when using the spell 
[Blink], accidentally got inserted into the side of a barn. A goblin shredder was used to remove her from said barn, but she lost a hand and a foot in the process. Her accident is chronicled in the book Blinking Rules & Regulations.
