Focused Malignancy is a conduit for affliction warlocks. It drops from Mordretha, the Endless Empress in the Theater of Pain and Artificer Xy'mox in the Sepulcher of the First Ones.

It can also be purchased from Zo'sorg in Oribos for 200 .

It can also drop from various Shadowlands rare spawns, world quests, and Covenant Callings, scaling to your item level.
