The Head of Nefarian is dropped by Nefarian in Blackwing Lair. Only one head drops per kill.

Like the 
[Head of Onyxia], Nefarian's head may be delivered either to King Anduin Wrynn in Stormwind for the Alliance or High Overlord Varok Saurfang in Orgrimmar for the Horde. Wrynn will send you to Field Marshal Stonebridge in the Valley of Heroes, while Saurfang will send you to Overlord Natoj near the gates of Orgrimmar.
Turning in the head grants everyone in the city the [Rallying Cry of the Dragonslayer].
The head is also involved in the chain to obtain the 
[Heart of Corruption]. Paladins working on this can take the head and 
[A Complete Copy of "Nat Pagle's Guide to Extreme Anglin'."] to Prince Tortheldrin in Dire Maul.
