The second Inconspicuous Note is found in the middle of the Scrapyard in Ulduar after solving the puzzle of the Scrapyard Lights. It holds the second clue for obtaining the

Solution[]
The War of the Shifting Sands took place roughly 1000 years ago in Ahn'Qiraj at the southern end of the Silithus desert. Until the bearer of 
[The Scepter of the Shifting Sands] struck the Scarab Gong to open the Gates of Ahn'Qiraj, the fortress and all of its denizens were sealed inside.
Three of those denizens are the dragons Arygos, Caelestrasz, and Merithra, found near C'Thun's chamber at the end of the instance. Get to them, then run past them into the always-unused room up the stairs to the right.
In that room on an altar is a Mind Larva. Interact with it to begin a 
[JewelCraft]-like minigame where the objective is to line up five brains vertically or horizontally. After completing the objective, the third Inconspicuous Note appears on the altar. Interact with it to move on.
