Destroy Levixus and take the Book of the Dead from his possession.
Deliver the Book of the Dead to Ramdor the Mad standing outside the Auchenai Crypt entrance of Auchindoun.

Go now, <name>. You will find Levixus on the uppermost ruins of Auchindoun. Enter there from the southern reaches of the Bone Wastes. He stands amidst a storm of souls...
Destroy him and take the book of the dead. Once you have the cursed book in your possession, destroy it! Destroy it so that such an atrocity can never again be made possible.

There are strategies out there that recommend kiting him to the Sha'tari Base Camp. Note: If Levixus mind controls you and you kill one of the defenders, it costs you 1250 rep with them, down to 0/3000 neutral. They are 70 lvl elites, so its pretty unlikely, but Levixus can also mind control them... (This note refers to an earlier version of the quest: the quest originally recommended a group of three, as Levixus was a level 66 elite demon and notoriously hard to kill because of his Mind Control and the infernals he summoned.)
