Before the fall of her house, Margrave Stradama was kind, talented, and knowledgeable. She was soulbound to Plague Deviser Marileth and confided her fears in him after the Primus's disappearance and the beginning of the anima drought, the latter of which had caused the House of Plagues' work to stagnate. The House of Rituals offered to provide the House of Plagues with anima and sent the necromancer Korrim the Cunning to assist in their experiments. Stradama and Marileth both suspected duplicity, but their house was in no position to refuse. As a last resort, Marileth devised a potion to protect his margrave if the need arose, but before he could craft it, the House of Plagues was annihilated. Stradama herself was driven mad from a mixture of twelve different airborne poxes and was transformed into a monstrous being of slime and bone.
With help from the Maw Walker, the now-insane Marileth later managed to complete his potion. Stradama appeared out of the slime below Marileth's laboratory alongside two Plaguefall Wards, but the potion succeeded in granting her and Marileth a brief moment of lucidity. Marileth was shocked at Stradama's fate, but the margrave told her "dear Marileth" that he had to forgive himself and that while she was lost, he had to endure. The margrave disappeared back into the slime and Marileth promised that he would endure for her sake. Moments later, the effects of the potion wore off, but the act of temporarily clearing Stradama and Marileth's minds awakened the Rune of Insight on the Blade of the Primus carried by the Maw Walker.
It was believed that Margrave Stradama was destroyed in the explosion that brought down her house. In truth the explosion transformed her into a monstrous new form, driving her mad, and leaving her at the heart of the fallen House of Plagues.
During the early alpha, Stradama's Adventure Guide entry read: "Transformed due to the explosion that destroyed her house, Margrave Stradama is now the source of the plague that continues to ooze across Maldraxxus."
