Patch changes

• Patch 5.0.4 (2012-08-28): Removed.
  
• Patch 4.0.1 (2010-10-12): No longer reduces the cooldown of Death and Decay (was a 5/10/15 sec reduction). Now increases Death and Decay damage by 10/20/30%.
  
• Patch 3.1.0 (2009-04-14): Moved from tier 1 to tier 2.
  
• Patch 3.0.2 (2008-10-14): Added.
