Myzerian's Head drops from Myzerian at the Obelisk of the Moon in northwest Uldum.
It begins the eponymous quest 
[30-35] Myzerian's Head.
Players are only capable of killing Myzerian after weakening him during [30-35] Shroud of the Makers, late in the Harrison Jones storyline. See Myzerian's article for tactics on defeating him.
