This item drops from the Lich King in the 10-man version of Icecrown Citadel.

Notes

The original owner of this dagger is not clear, though it probably belonged to either Anasterian or to Kael'thas Sunstrider. If it was Anasterian's, it could have been obtained when The Scourge attacked Silvermoon City to get to The Sunwell; Anasterian was killed during this war. However, the elf population of Silvermoon was considered High elf at the time, so Anasterian could never be the Blood King. More likely, this dagger belonged to Kael'thas, who decreed himself and his subjects blood elves in memorial to those that fell to the Scourge. Before he became The Lich King, Arthas was a rival with Kael'thas for Jaina Proudmoore, and the armies of the two clashed in Northrend as Arthas went to The Frozen Throne; Arthas probably would have obtained the dagger then. This, however, raises issues of its own, as Kael'thas may never have been crowned officially.
