Malfurion, Tyrande and Maiev can find one in Balancing the Scales, chapter 5 of that campaign.
Malfurion and Maiev can also find one in The Ruins of Dalaran, chapter 7 of that campaign.
Patch changes
Patch 1.10 (2003-07-03): Tomes are now "powerups". This means that if you right-click on them with a Hero selected, the Hero will now automatically use them rather than pick them up.
