Contained in Unforged Runic Breastplate near Goraluk Anvilcrack in Upper Blackrock Spire. It can be 'opened' once per instance; if the opener does not have the quest, there will be no loot.

As a quest objective

The subject of this section has been removed from World of Warcraft.

It was required in the blacksmithing-related quest  [60D] The Demon Forge. Characters that were still on the quest were able to loot the item, but no longer able to complete it.
