This item drops from the Lich King in the 10-man version of Icecrown Citadel.

Notes

The Menethil family was the royal line of Lordaeron until it fell to The Scourge. This hammer either belonged to King Terenas Menethil, or was once owned by Arthas Menethil before he became the Lich King. Since the mace is intended for melee classes it is probably a safer guess that Arthas once owned this, as the Prince was once a renowned paladin.
